Confirm Direct Debit Registration

Once you have received your confirmation email that your direct debit has been set up, please complete this form including the names of all of the cadets that are covered by the direct debit payment. Please also include the email address that you used when you registered for Direct Debit.



By submitting this form you agree that...

  1. You will notify Swansea Sea Cadets, giving 28 days notice, if you wish to cancel, or amend, the Direct Debit payment.

  2. You will be responsible for cancelling the Direct Debit payment in the event that a cadet leaves the unit, and, that Swansea Sea Cadets will not refund any payments in the event of a delay in cancelling a Direct Debit when a cadet leaves the unit.

  3. Swansea Sea Cadets can use the contact information stored in Westminster to contact me about payment problems.
